Hans Morefield
Hans Morefield is the CEO of CHESS Health, a leading provider of evidence-based digital health solutions focused on substance use disorder serving the public sector, health plans, and provider organizations. Founded in 2014 to commercialize an evidence-based smartphone app developed at the University of Wisconsin, CHESS Health has expanded its platform’s focus and capabilities under Han’s leadership to serve the entire spectrum of SUD management from proactive prevention and linkage to care, to supporting treatment and sustained recovery with human support from a 24/7 team of peer recovery specialists. Prior to joining CHESS Health, Hans had senior management roles at other healthcare technology companies, with a focus on care coordination and patient access. Kim O’Connor, LCSW
Kim O’Connor is the Founder, Clinical Director and Psychotherapist of M Kim O’Connor & Associates. Kim is a clinical social worker and psychotherapist with over 30 years of experience working with youth and adults in a variety of settings including hospitals, mental health centers, not-for-profits, and schools. In addition to her private psychotherapy practice, Kim is an instructor in the NYU School of Social Work Master’s Program. Kim serves on the Open Door Foundation Board of Directors and is actively engaged in her community, supporting the well-being of youth and families through volunteer work with a variety of local youth and family organizations. Kim is a graduate of New York University, Silver School of Social Work.

Lindsay Farrell, President and CEO of Open Door Family Medical Center
Ms. Farrell has dedicated her professional life to high-quality community-based care, efficient care delivery, and population health management to assure a more equitable health care system for us all. Home-grown at Open Door where she originally served as a volunteer, Ms. Farrell has also served as Open Door’s Director of Operations and Director of Development.
During Ms. Farrell’s tenure as President and CEO, Open Door has grown significantly and doubled the number of people served. Today, Open Door is accredited by the Joint Commission, recognized by the National Committee on Quality Assurance as a Level 3 Patient-Centered Medical Home and has won the prestigious national Davies Award from HIMSS, the Healthcare Information Management Systems Society.
Ms. Farrell is a member of the Board of Directors of MVP Health Plan in Schenectady, the Community Health Care Association of New York State, the Taconic Health Information Network and Community (THINC) and the Health Center Controlled Network of New York. She is also a past Chair of the Westchester Women’s Agenda.
Ms. Farrell is a graduate of St. Lawrence University and received her Master’s in Business Administration from the Lubin School of Business at Pace University. She is a Fellow of the American College of Medical Practice Executives. She has received the Sol Feinstein Community Service Award from St. Lawrence University, the Betsy Cooke Grassroots Advocacy Award from the National Association of Community Health Centers, and has been named as one of Westchester County’s most influential residents by Westchester Magazine.

On November 9th, 350 individuals TUNED-IN to keep our communities “tuned-up” and healthy! We appreciate everyone’s generosity in supporting Open Door’s critical care coordination services that keep our patients healthy and on track outside the exam room. A special thanks to our honorees and the incredible musicians who came together to support good health, equity and community.
PowerUP for Open Door
On May 4, 2017 we hosted PowerUP at the beautiful Tappan Hill Mansion in Tarrytown. Over 200 supporters attended to celebrate the grit and grace of those who have shaped our community. StarPower keynote speaker, Linda Kaplan Thaler, inspired guests to empower others by harnessing their own passion and perseverance. and PowerHouse honoree Shirley Acevedo Buontempo received the Changemaker Award for her transformative work with first generation Latino college students.
On October 21, 2016 we held our annual Fall Fest at Brae Burn Country Club in Purchase. A standing room only crowd raised their paddle to support quality health care as a right for everyone and not a privilege reserved for those who can afford it. Together, we kicked off our shoes, line danced, and honored Kate and Alan Sanzenbacher, Barbara and Tom Cleveland, and June Keenan for their leadership and advocacy on behalf of community health care.
