Open Door Receives National Recognition

Open Door Family Medical Center has been awarded Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) at all six of its medical sites – in Ossining, Brewster, Mamaroneck, Mount Kisco, Sleepy Hollow, and Port Chester.  

The Patient Centered Medical Home is a model of care that puts patients at the forefront. Research shows that PCMHs improve quality and the patient experience, increase clinical staff satisfaction, and reduce health care costs. Practices that earn this recognition have made commitments to continuous quality improvement and patient-centered approaches to care.  

NCQA’s PCMH recognition program is the most widely adopted PCMH evaluation program in the United States. To receive this recognition, Open Door was required to complete criteria in six concept areas:  

  • Team-based care and practice organization  
  • Knowing and managing patients  
  • Patient-centered access and continuity  
  • Care management and support  
  • Care coordination and care transitions  
  • Performance measurement and quality improvement  

Open Door first received Level 3 NCQA PCMH in 2009. To retain the PCMH recognition, Open Door must reapply for consideration on an annual basis and submit to NCQA evidence of ongoing quality improvement.
